FAQ - Disability Income and Life Insurance

Our disability income plan provides $450 per month after a 30 day exclusion period if you are unable to work due to a disabling condition.  Payments continue until age 65 or until you return to work. Life insurance premium is waived during the period of disability.

Life Insurance Coverage - $64,000, while employed or until age 65 if you are under waiver of premium because of a disability.

Our Union administers both plans so claims are filed directly with the Union Office.

MetLife Beneficiary Form (LINK TO FORM)

Our contractually bargained Life Insurance and Disability program is through MetLife. These insurance plans are administered by our Union. Please fill out a beneficiary form if you  need to  change  beneficiaries or never filled one out when hired.  

If you have never filled out a beneficiary form, and a life insurance claim is needed, then the beneficiaries would be determined by the statutory order of beneficiaries  (spouse, children, parents, siblings, etc)

If you would like to specify beneficiaries then you will need to fill out the attached MetLife Beneficiary Form and send it to “CHTU” through  school mail or scan to office@chtu.org.  Although the form is a fillable PDF it will still need to be printed so that you can sign the form.

WHAT TO KNOW ABOUT FILLING OUT THE METLIFE BENEFICIARY DESIGNATION FORM

Section 1: your info

Section 2: YOU DO NOT NEED TO FILL OUT ALL OF THESE SECTIONS.  ONLY THE ONES THAT APPLY:

  1. Individual beneficiaries

Primary beneficiaries are the one or more individuals  among whom you want to assign or divide  the benefit.  If you have more than 3 beneficiaries, then continue listing on a new copy of page 1.  Make sure percentage adds to 100%

Contingent beneficiaries: if you choose to fill out this section,  it would only apply if all of your primary beneficiaries are not living at the time the claim is made.  If you need more than 3 spaces then add a copy of page 2 listing those names.

  1. Living Trust.  If you have a living trust (not common)
  2. Testamentary trust created in the insured’s will.
  3. Insured’s estate.  This leaves it to your estate to split up based on your will or sorted by the statutory order  of beneficiaries.  Use it if you don’t want to duplicate what is already in your will.
  4. Charity/Organization. If you prefer to not leave your benefit to a person.

Section 3:  A signature is required for the form to be used.  Print and sign.  Send the form the “CHTU” in office mail or scan to office@chtu.org.  MetLife does not manage the beneficiary forms.  The school district does not manage the beneficiary forms.  CHTU keeps these records and has always filed life insurance claims.
