CHTU Update: Preventing a Needless Tragedy

Dear Colleagues,

Recently my family experienced  a health crisis that I wouldn’t wish on my worst enemy.  Fortunately, my husband survived and is now living with a manageable but serious chronic condition. The lessons I learned are that tomorrow is never promised, and know your health warning signs before it’s too late.  There is so much for us to learn about our health care benefits and there are some benefits available that I never thought about until it became my reality last week.  If you don’t have a primary care physician (PCP) or you haven’t been to the doctor in a while please sign up with a PCP and schedule an appointment.  We have negotiated  great benefits and a very large network of coverage. If you are living with a chronic health condition, like asthma or diabetes, our MMO  Chronic Condition Management program provides education and support to help you achieve your best overall health. This program can offer support through digital health coaching, telephonic coaching, and home-based monitoring devices, depending on your condition and preference. You will also have access to specially trained health coaches who can provide valuable knowledge about your condition(s). This program is available at no out-of-pocket cost  to you.

You can  call 1-800-590-2583 (TTY 711) Monday through Friday, 9AM-4PM EST to check eligibility and enroll, or leave a confidential voicemail 24/7. You can also call this number if you are already enrolled and need to talk to your health coach.

If you do not want to enroll in the Chronic Condition Management Program, you can still get a glucose monitor covered under your regular MMO benefits. For your highest level of benefits to be considered you will want to go to a network Durable Medical Equipment (DME) Supplier. You can find network providers through your My Health Plan account by selecting Find a Provider under Quick Links on your Dashboard.

Under our regular plan benefits for DME services, claims in-network are first subject to your calendar year deductible of $100 individual/$200 family and then considered at 90% of the allowed amount. Your 10% coinsurance applies once your deductible is met and will keep applying until you meet your coinsurance out-of-pocket limit of $400 individual/$800 family.Our  plan deductibles and copayments are based on a  calendar year.
